YUG Wellness Liability Waiver & Release

I understand that yoga includes physical movements and as is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. In participating in the services offered at YUG Wellness, I agree that I do so at my own risk. In consideration of being permitted to use the facilities and equipment located at YUG Wellness, I understand that YUG Wellness shall not be liable for the loss or theft of, or damage to, the personal property of a guest or member. I assume full responsibility for any injuries or damages which may occur using studio facilities and equipment, and I hereby fully and forever release and discharge YUG Wellness, its owners, employees, and members from any and all claims, demands, damages, rights of action, or causes of actions, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting or arising out of my or a member's use or intended use of the facilities and equipment, on or about the premises of YUG Wellness. I acknowledge that I am fully liable for all medical expenses as a result of any injury incurred during my participation in the services offered at YUG Wellness. 

Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. By signing, I affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness program. In addition, I will make the instructor aware of any medical conditions or physical limitations before class. If I am pregnant, become pregnant or I am post-natal or post-surgical, my signature verifies that I have my physician's approval to participate. I also affirm that I alone am responsible to decide whether to practice yoga and participation is at my own risk.

I understand and acknowledge that I have the right to consensual practice and that consent is a requirement of tactile feedback and physical touch for all services at YUG Wellness. I understand that I will be given the option in every class to anonymously give consent for hands on adjustments.

COVID-19 WARNING, DISCLAIMER, AND CLIENT WARRANT

YUG Wellness has put in place preventive measures to reduce the spread of COVID-19; however, YUG Wellness cannot guarantee that you will not become infected with COVID-19. Further, attending the services at YUG Wellness could increase your risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the services at YUG Wellness and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the services offered at YUG Wellness may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees and other participants. I knowingly and freely assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at YUG Wellness` services.

I acknowledge and agree to uphold the condition of entry to immediately notify YUG Wellness if I no longer comply with any of the foregoing statements and agree to not enter any YUG Wellness locations:

I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or loss of taste or smell.

I have not traveled internationally within the last 14 days.

I have not traveled to a highly impacted area within the United States of America in the last 14 days.

I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

I have not been diagnosed with Coronavirus/COVID-19 and not yet cleared as non contagious by state or local public health authorities.

I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19
